Welcome to the London West Health Centre

In order to maximize the one hour set aside for your initial consultation with Dr. Dronyk, we ask that you please complete all intake forms either downloaded, e-mailed, mailed or faxed to you. If possible, faxing, e-mailing or dropping off the forms to the office prior to your visit will allow Dr. Dronyk time to review your information, possibly saving more time for your initial visit. If for any reason you are unable to fill out the forms prior to your appointment, please arrive at least 20 minutes before your appointment time to complete all forms. If you have completed the forms, but not forwarded them to the office prior to your appointment, please remember to bring the forms with you.
